These cells later form the functional sensory cells in the inner ear and neurons of the eighth cranial nerve. To complement the 2005 Annual Clinical Focus on medical genomics, AFP will be publishing a series of short reviews on genetic syndromes. This series was designed to increase awareness of these diseases so that family physicians can recognize and diagnose children with these disorders and understand the kind of care they might require in the future. The second review in this series discusses fetal alcohol syndrome and fetal alcohol spectrum disorders. As is the case with any medical condition, sound clinical judgment was exercised in assigning diagnoses in the FASD continuum.

Alcoholic beverages contain different percentages of ethanol, all of which are teratogenic. One form of long-lasting damage that can be caused by alcohol is fetal alcohol syndrome (FAS), part of the umbrella term fetal alcohol spectrum disorders (FASDs) (British Medical Association (BMA), 2007a; National Institute or Health and Care Excellence (NICE), 2019). However, the prevalence of FASDs is difficult to determine (Schölin et al, 2021a). Neurobehavioral disabilities in FASD include deficient global intellectual ability and cognition, and poor behavior, self-regulation, and adaptive skills.

Screening for Prenatal Alcohol Exposure and Selection of Participants

Almost all experts recommend that the mother abstain from alcohol use during pregnancy to prevent FASDs. As the woman may not become aware that she has conceived until several weeks into the pregnancy, it is also recommended to abstain while attempting to become pregnant. The rates of alcohol use, FAS, and FASD are likely to be underestimated because of the difficulty in making the diagnosis and the reluctance of clinicians to label children and mothers. Some have argued that the FAS label stigmatizes alcohol use, while authorities point out that the risk is real. Fetal alcohol spectrum disorders (FASDs) are a group of conditions that can occur in a person exposed to alcohol before birth.

During pregnancy, VE deficiency causes several developmental abnormalities and behavioral deficits, which may cause oxidative stress of the rapidly growing embryo 78. This indicates that the severity of ophthalmological involvement in FASD may vary drastically. Regardless of the primary cause behind these findings and whether these individuals reach the cut-off total score or not, abnormalities detected with the FASD Eye Code are important and need clinical management. This study further supports the utility of implementing a complementary ophthalmological diagnostic tool in the FASD diagnostic workup. Diagnosing FASD in children and adults plays a critical role in identifying co-morbid and co-occurring conditions and accessing vocational support, housing and financial assistance, psychological interventions, and specialized legal counseling. These services may attenuate the impact of commonly co-occurring disabilities, including academic failure, social problems, criminal behavior, alcohol and drug use disorders, and employment difficulties. The prevalence of mild sensorineural hearing loss in children diagnosed with FASD (16dB hearing-level or greater)  was  not  higher  than  expected  in  this  age  group.

About 25-50% of fetal alcohol syndrome symptoms children with FASD suffer from strabismus, which can be corrected with patching of the normal eye to correct the deviation if identified early. Other problems may include cataracts, which can be corrected surgically, and nystagmus. The cardiac abnormality seen in FASD is most typically a ventricular septal defect.

This developmental period includes optic primordium specification and neurogenesis events. Treatment during time windows before or after 12–24 hpf did not cause as severe defects as those produced by treatment during 12–24 hpf. Also, higher ethanol concentrations during this sensitive period induced severe lamination defects and cyclopia 43.
